INDICATIONS
Treatment of Influenza
VIRENZA is indicated for treatment of uncomplicated acute illness due to influenza A and B virus in adults and pediatric patients 7 years and older that has been symptomatic for no more than 2 days.
Prophylaxis of Influenza
VIRENZA is indicated in adults and pediatric patients 5 years of age and older for prophylaxis of influenza.
VIRENZA is not recommended for treatment of patients with underlying airways disease (such as asthma or chronic obstructive pulmonary disease)
DOSAGE AND ADMINISTRATION
VIRENZA is for administration to the respiratory tract by oral inhalation using Revolizer device only.
Patients scheduled to use an inhaled bronchodilator at the same time as VIRENZA should use their bronchodilator before taking VIRENZA.
Treatment of Influenza
The recommended dose of VIRENZA for treatment of influenza in adults and pediatric patientsaged 7 years and older is 2 inhalations (one 5-mg capsule per inhalation for a total dose of 10 mg) twice daily (approximately 12 hours apart) for 5 days.
Two doses should be taken on the first day of treatment whenever possible provided there is at least 2 hours between doses. On subsequent days, doses should be about 12 hours apart (e.g., morning and evening) at approximately the same time each day. There are no data on the effectiveness of treatment with Zanamivir when initiated more than 2 days after the onset of signs or symptoms.

KARACHI: Senior health officials working amid a swine flu alarm in the city have put their faith in antiviral medicines being provided to them, ignoring their health facilities’ lack of capacity for a fully-fledged treatment of patients reporting with a severe infection of the H1N1 virus.

Sources added that 35,000 Tamiflu tablets would be supplied to at least seven large hospitals run by the Sindh and city governments within a couple of days. However, the preparation of isolation wards in public sector hospitals for treatment and management of patients complaining of severe respiratory problems and chest infections would not be done anytime soon.

These hospitals would not be able to establish such wards with ventilators required exclusively for swine flu treatment.

The federal health ministry’s Director-General Dr Rasheed A. Jooma told Dawn on Friday that the swine flu death toll in the country had risen to eight. The deaths have been reported from Peshawar, Lahore, Karachi and Faisalabad, according to him. The National Institute of Health (NIH) in Islamabad, he said, had informed him that 37 persons were positive among the 190 tested for swine flu so far.

Pointing out that there had been a rise in swine flu cases being reported in Karachi, he insisted that the virus had been carried by someone coming from abroad.

The focal person on swine flu for Sindh, Dr Shakeel A. Mullick, said that the NIH had so far confirmed five swine flu cases in the province. Four of the patients had already tested positive by the Aga Khan University Hospital (AKUH) laboratory while the other one was deported from Oman and quarantined at the federal health ministry facility near Karachi airport.

Why is swine flu (H1N1) now infecting humans?

Many researchers now consider that two main series of events can lead to swine flu (and also avian or bird flu) becoming a major cause for influenza illness in humans.
First, the influenza viruses (types A, B, C) are enveloped RNA viruses with a segmented genome; this means the viral RNA genetic code is not a single strand of RNA but exists as eight different RNA segments in the influenza viruses. A human (or bird) influenza virus can infect a pig respiratory cell at the same time as a swine influenza virus; some of the replicating RNA strands from the human virus can get mistakenly enclosed inside the enveloped swine influenza virus. For example, one cell could contain eight swine flu and eight human flu RNA segments. The total number of RNA types in one cell would be 16; four swine and four human flu RNA segments could be incorporated into one particle, making a viable eight RNA segmented flu virus from the 16 available segment types. Various combinations of RNA segments can result in a new subtype of virus (known as antigenic shift) that may have the ability to preferentially infect humans but still show characteristics unique to the swine influenza virus (see Figure 1). It is even possible to include RNA strands from birds, swine, and human influenza viruses into one virus if a cell becomes infected with all three types of influenza (for example, two bird flu, three swine flu, and three human flu RNA segments to produce a viable eight-segment new type of flu viral genome). Formation of a new viral type is considered to be antigenic shift; small changes in an individual RNA segment in flu viruses are termed antigenic drift and result in minor changes in the virus. However, these can accumulate over time to produce enough minor changes that cumulatively change the virus' antigenic makeup over time (usually years).

H1N1 Flu (Swine Flu)

Swine flu is an infection caused by a virus. It's named for a virus that pigs can get. People do not normally get swine flu, but human infections can and do happen. The virus is contagious and can spread from human to human. Symptoms of swine flu in people are similar to the symptoms of regular human flu and include fever, cough, sore throat, body aches, headache, chills and fatigue.

  • Avoiding touching your eyes, nose or mouth. Germs spread this way.
  • Trying to avoid close contact with sick people.
  • Staying home from work or school if you are sick.

Take steps to contain swine flu, K Rosaiah to officers

Andhra Pradesh chief minister K Rosaiah today asked the health department officials to take all steps to contain the spread of infectious diseases including swine flu in the state.
Rosaiah held a review meeting on all infectious diseases with minister and Officials at Secretariat in the evening today, a release from the CMO said.
He said the government is fully geared up to meet the situation and to contain the spread of swine flu and other infectious deceases. The Chief Minister asked the Finance Department to release funds as required by the Commissioner, Health towards this.
Rosaiah asked the minster for health and special chief secretary and commissioner, Health to hold a video conference with district Collectors and other officials concerned tomorrow from Secretariat and provide the necessary instructions, the release said.
The chief minister who expressed concern at the media reports about the incidents of swine flu and other viral diseases told the officials to act immediately and from Rapid Response Teams (RRT) consisting of doctors and paramedical staff and dispatch them to districts.
Rosaiah was told that the state Epidemic Cell is functioning round-the-clock, state RRT is formed for investigation of outbreaks of epidemics and supply of drugs and disinfectants are being streamlined, weekly reviews are held daily reporting is done on malaria, H1N1, fevers and other diseases, it said.
As far as H1N1 is concerned, 83 treatment centres have been established for swine flu management in the state, technical committee is formed with professor and general medicine, microbiology, SPM, chest physicians and state Nodal Officers to review each positive case and each death case, it said.
The government is providing adequate doses of oseltamivir tablets, 80,400 doses for vaccination of medical staff and necessary medical and other equipment being provided, the release said.
Officials said over 225 positive cases of H1N1 have been reported in the state since January. About 25 people have succumbed to the disease so far.

US stocks douse early losses on consumer sentiment

Wall Street reversed its sharp fall on Friday, trimming losses after consumer sentiment in the US partly offset a worse-than-expected an annualized 2.4% US GDP in the second quarter. Question marks to intensify the downside pattern arose, as investors were conflicted with the string of mixed indicators.

However, given the large relevancy on the economic growth picture in the US, the reported downbeat rise continues to limit any efforts to keep benchmark indexes afloat. The Dow Jones Industrial Average was down 0.43%, yet far from its daily low, the Nasdaq Compsitr declined 0.58% and the S&P 500 fell 0.45%.

New Zealand: RBNZ hikes rate to 3.00% in line with expectations

The Reserve Bank of New Zealand decided to raise the interest rate to 3.00% from its previous 2.75%, matching the expectations of most surveyed economists. While the hike is hawkish, the June 10 statement by the RBNZ made it seem more increases were less likely citing weak economic fundamentals like slow retail and declining housing and net immigration.

RBNZ Interest Rate Decision is announced by the Reserve Bank of New Zealand. If the RBNZ is hawkish about the inflationary outlook of the economy and rises the interest rates it is positive, or bullish, for the NZD.

Forex: USD/CAD jumps to 1.0360 after Canadian GDP

Dollar retreat from 1.0390/95 resistance area, tested on Thursday for the fourth time, has been halted at 1.0310 session low, as the Dollar jumped to 1.0365 after the release of Canadian GDP Data.

The pair is approaching 1.0390/95 resistance (Jul 27/28/29 highs), with next resistance levels above here, at 1.0445 (Jul 23 high) and 1.0505 (Jul 21/22 high). On the downside, support levels lie at 1.0300 (Jul 28/29 lows), and below here, 1.0255 (Jul 27 low) and 1.0180 (Jun 22 low).

Canadian economy advanced at a 0.1% pace in May, following a flat performance in April, below market expectations of a 0.2% growth.

Oil drifts off 11-week high

NEW YORK (CNNMoney.com) -- Oil prices drifted lower this week, edging off last week's 11-week high above $79 a barrel. But trading remained rangebound as investors balanced strong corporate earnings against ongoing jitters over the economic recovery.
Prices tumbled midweek to below $77 a barrel before regaining some ground on Thursday. Absent any big catalyst, analysts expect that type of action to continue.
The first round of pressure on prices came Tuesday after the Conference Board reported that its consumer confidence index sank in July to the lowest reading since February."Oil has rallied $8 since the beginning of the month, and I think this drawback is on slightly weaker economic news in the U.S. that has stirred up a whole lot of talk about a double-dip recession," said James Cordier, president of Liberty Trading Group.
Oil prices continued to slide Wednesday after an inventory report from the Energy Information Administration showed a surprise build in crude supplies.
The report showed that crude stocks climbed by 7.3 million barrels last week, surprising analysts who were expecting inventories to fall by 2.3 million barrels, according to a consensus estimate collected by energy information provider Platts.
Cordier said worries about dwindling demand for crude will continue as the peak driving season ends and brings around the fall period, when oil consumption is seasonally the lowest. .
Oil rebounded 1.8% Thursday to $78.36 as the dollar dropped to a 11-week low against the euro. Crude, like other commodities, is priced in dollar, and a weaker greenback boosts prices.
